Transitions LifeCare is hiring immediately. As one of the communitys industry leaders, we provide exceptional home health, hospice, and palliative care services throughout our 8-county territory. Transitions LifeCares goal is to care for individuals and families with compassion, support, and expertise as they navigate the end of life or life changing illness.The schedule for this position is 3 shifts per month 7:00pm-7:30am.Why you should join Transitions:**Shift Differential for weekday nights and weekends**A Successful Hospice Home RN has the following:Caring and compassionate attitude with true concern for peopleExcellent interpersonal skills: ability to connect with patients, families, and internal/external partnersStrong ability to problem-solve and communicate complex issues; ability to work collaboratively with all co-workersExhibit ability to make responsible choice and decisions that are in patients best interestResponsibilities:Plan, provide, supervise and document safe, competent and compassionate hospice nursing care for patientsWork collaboratively with the IDG Team and other internal/external partners in achieving patient care goalsUse professional nursing judgement to individualize the plan of care based on assessment of the unique needs of each patient.Maintain comfort, privacy, and dignity of our patientsDevelop working knowledge of all federal, state and company regulations, policies, and procedures; ensure complianceRequirements:Registered Nurse with current NC license; BSN preferred.Must have reliable transportation to travel to, enter and function in the homes/facility of any patient/family in the service area to assess and provide patient care.Must submit and pass all applicable pre-employment and annual tests and meet all training and other applicable state and federal guidelinesWhat you can expect from us:Hiring manager or Human Resources will contact you within 1-3 business days of your application submissionComplete 1-2 interview(s) with the hiring manager via virtual video, or in-personUpon receiving the job offer, review job description, and complete new-hire tasks and paperwork (via UKG)Complete New-Hire Orientation and clinical skills trainingReceive continuous training from our team as you continue to provide exceptional care to our patientsEssential Functions:To perform this job successfully, an individual must be able to perform each essential duty.
